APPLICATION NETHERLANDS PASSPORT

In principle a passport application should be made in person, including minors (both in case of minors to be registered in a parents passport as well as minors to be issued a separate passport). Opening hours of the Consular Section: Monday to Friday from 9.00 a.m. until 1.00 p.m.

As of 28 August 2006 new requirements for the passport photograph apply: please check at www.paspoortinformatie.nl or consult the Embassy.


Please submit the following documents:

For all applications:
- two passport photo's (for new requirements check www.paspoortinformatie.nl)
- fully completed and signed application form, available at the Consular Section
- current passport
- valid residence permit (in case you do not have the Lebanese nationality)
- any other Netherlands or foreign passport(s) or identity card(s)
- a recent family extract

In case you want to have the name of your spouse mentioned in the passport:
(only possible in a new passport)
- marriage concluded in the Netherlands: marriage certificate from the municipality
('huwelijksboekje' is not accepted)
- marriage concluded in Lebanon: recent marriage certificate from the church/ religious
court plus marriage-registration by the Lebanese Ministry of Interior
- marriage concluded in third country: foreign marriage certificate legalized by the Dutch Embassy in that country

Minors
Registering of a minor under 16 years of age in the parents passport, or application for a separate passport: submit the documents abovementioned plus:
- birth certificate of the child (translated and legalised)
- permission of both parents (in principle personal presentation of both parents is required)
- the valid passport of both parents
- a recently issued marriage certificate of the parents (see above)
- in case one of the parents has the custody over the child, the passport of the legal custodian and the legalised custody papers should be submitted.

Please note that all foreign documents must be translated into English and legalised by the authorities concerned.
